We are in the process of undertaking a 360-degree review of the specified individual and are seeking feedback on their performance from several respondents across the company. You have been selected to be a rater in their 360 process and provide feedback because the participant values your perspective and input.

Your responses are highly valued and will enable us to gather meaningful feedback about their performance. 

Anonymity
To support you in providing honest feedback to the participant, the 360 process is anonymous. The feedback report will not identify you individually as the provider of any specific ratings about the participant. 

Time Requirement
The 360 questionnaire will take approximately 10 minutes to complete.

All questions are mandatory except the questions which require additional comments.
The ‘next’ and ‘submit’ buttons only appear if you have selected an answer for each question (comments question excluded). If the buttons do not appear at the bottom of the section, please go back and ensure you have a selected an answer for every question.

We are in the process of undertaking a 360 degree review for each of our senior managers.   

 

A 360-degree review is a tool used to measure skills, competencies and behaviours. It uses a survey to combine an individual’s self-assessment with feedback from direct reports, peers, management and colleagues in one report. Results are then presented anonymously to the person surveyed.

 

This survey is being co-ordinated by (YOUR BUSINESS NAME HERE) and administered by an independent provider – HR Profiling Solutions. (CLIENT NAME) will not have access to any information that could potentially identify you as an individual.

 

The survey will go live on (INSERT DATE HERE) and close on (INSERT DATE HERE).

 

Allow approximately 15-30 minutes to complete it.

 

Later today, you will receive an email from HR Profiling which will contain information and links to the survey.

 

Please note this survey is important for our managers’ development, so your prompt and honest response is much appreciated.
